Transition Care at Martin Luther Homes

We are excited to have partnered with Eastern Health to provide a transition care program (care after a hospital stay) at Martin Luther Homes. This service will commence by the End of June 2019 and occupy 30 beds in our new extension. 

Transition care is for older people who have been in hospital and need extra help to recover. The program can be provided for up to 12 weeks. This may be extended to 18 weeks if one has been assessed as needing more help while in transition care. From transition care people will be discharged home or receive respite care or residential aged care.

Martin Luther Homes launches STRC program

Martin Luther Homes now provides Short-Term Restorative Care (STRC). Short-Term Restorative Care is a new goal-oriented program that provides early intervention care for older people.

It aims at keeping the clients independent by reversing and/or slowing functional health decline. Its ultimate goal is to help recipients to regain their autonomy and to prevent them from relying on long-term care.

The STRC is delivered in the form of a tailored, multidisciplinary package of services. These include physiotherapy, social work, nursing support, personal care, and the provision of new technologies that make your life easier. The STRC service is limited to a maximum of 56 days and can be delivered at Martin Luther Homes’ facilities or in the comfort of the client’s home.
