Martin Luther Homes offers Support at Home (short: SAH), either at our modern facility or in the comfort of your own home. SAH helps recipients to regain their autonomy and prevent them from relying on long-term care. The SAH services are tailored to the individual and can include physio, social work, nursing support, and personal care.

What is SAH?

Support at Home (short SAH) is a new program that provides early intervention care for older people. It aims at keeping you independent by reversing and/or slowing functional health decline. Its ultimate goal is to help recipients regain their autonomy and to prevent them from relying on long-term care.

The SAH is delivered in the form of a tailored, multidisciplinary package of services. These include physiotherapy, social work, nursing support, personal care, and the provision of new technologies that make your life easier.

The SAH service can be delivered at Martin Luther Homes’ facilities in Melbourne’s East or in the comfort of your own home.

How is SAH financed?

A range of services is financed by the Federal Government, and if you are eligible, you are expected to contribute to the costs, if you can afford to do so.

How to access SAH?

Your access is based on an aged care assessment with an Aged Care Assessment Team (ACAT) that you can arrange by contacting MyAgedCare on 1800 200 422 or directly by clicking HERE.

You can access SAH services if you are:

  • an older person
  • slowing down mentally and/or physically
  • not interested in long-term care
  • interested in remaining as independent as possible
